EMERGENCY RENTAL ASSISTANCE PAYMENTS
To access Emergency Rental Assistance Payments, a referral from a Medical Social Worker is required. Payments will only be approved and disbursed for eligible landlords and tenants.
Note for landlords: To receive financial assistance, landlords must complete a rental proceeds agreement and provide the latest W-9 form.

VOUCHER-BASED HOUSING
RENTAL PAYMENTS
Ongoing housing subsidy payments are available through the Emergency Rental Assistance Program. To access these payments, a referral from a Medical Social Worker is required. Approval and disbursement are strictly for eligible landlords and tenants.

ELIGIBILITY BASED UTILITY
ASSISTANCE PAYMENTS
To qualify for utility payment assistance, here's what you need to do:

Eligibility Assessment: Be willing to participate in an eligibility assessment with a program social worker.

Medical Documentation: Have a qualifying medical diagnosis and provide supporting medical documents.

Financial Hardship: Show proof of unexpected financial hardship, including documents that verify your situation.

Payment Details: Assistance amounts will be based on the emergency date, the costs of resolving your hardship, and the billing cycle affected.

CASE MANAGEMENT SERVICES
Our case management services are here to help you achieve greater independence in your health and housing stability. Upon referral, you will go through a screening process to determine eligibility.
If you're part of the Housing Assistance Fund program, you'll receive supportive case management specifically designed to help you meet your goals related to maintaining your health and stable housing.
A referral from a Medical Social Worker is required.
